Welcome to Collinsville Cardinal Athletics!

The Collinsville Athletic Department would like to thank you for taking the time to complete the forms required by the OSSAA and Collinsville Public Schools in order to participate in the athletic program.

All student athletes 7-12th grade that plan to participate in athletics each school year are required to complete all forms. Parents and students are required to read the OSSAA forms as they pertain to them. This year’s paperwork will be completed online to make the process convenient for all families.

CHS Athletics and the OSSAA require the parent/legal guardian and student athlete to complete the following electronic forms.
OSSAA Eligibility Form—Electronic Form Drug Consent Form (9-12th grade only) —Electronic Form
Athletics Medical Insurance Waiver—Electronic Form
Concussion Acknowledgement—Electronic Form
Sudden Cardiac Arrest—Electronic Form
Gifting Form—Electronic Form
Biological Sex at Birth Affidavit– Electronic Form
Over the Counter Medicine– Electronic Form
There will be a $25 Activity fee charged to all 9-12 grade athletes. That will be collected by the athletic office or coaches.

The only required form that cannot be done electronically is the annual physical and medical history forms. Parents will be able to download and print a physical and medical history form during the electronic session or may find them on any of the Cardinal Websites. The physical/medical history forms must be completed by a parent and a physician. The physical must be dated after May 1 for the corresponding year. The physical and medical history forms should be turned in to the coach, CHS Athletic Offices. It is the recommendation of the Collinsville Athletic Department that you make a copy of the physical for your files.
